Central problem addressed by the project: --transient pulmonary endothelial dysfunction with significant morbidity and mortality following cardiopulmonary bypass in patients with congenital heart disease --unknown relation of preoperative to postoperative pulmonary endothelial dysfunction Objectives: --examination of the components of pulmonary endothelial dysfunction before and after heart surgery Content: 1 design: --single centre prospective cross-over randomized study of the effects of various agents 2. patients: --patients with pre-operative pulmonary hypertension at cardiac catheterisation and following heart surgery 3. measurements: --oxygen content hemodynamic pressure measurements for shunt and oxygenation calculatgons --cardiac output and pulmonary vascular resistance using a dedicated resperatory mass spectrometry --cyclic GMP levels as an indirect marker of nitric oxide production 4. two cross-over protocols containing different chronological application of: --no substance (baseline) --infusion of substance P for examination of endogenous release of NO --infusion of L-Arginin for examination of NO-mediated response --inhaled NO for examination of sensitivity of pulmonary vascular smooth muscle Summary: The project proposes to examine the potential components of pulmonary endothelial dysfunction in patients with congenital heart disease before surgery, and how they are modified in the same patient afterwards in order to reduce morbidity and mortality associated with postoperative pulmonary hypertension.
