Накратко на български
Психосоциалните и заобикалящите фактори влияят върху появата на психотични симптоми при хора с деменция. Разбирането на тези причини помага за създаването на методи за грижа без използване на лекарства, което подобрява качеството им на живот.

Final Activity Report Summary - NIPID (Psychotic symptoms in dementia)
This study addresses a critical aspect of quality of life of persons with dementia in general and specifically, those of them who manifest psychotic symptoms. The objective of this study was to elucidate the psychosocial and environmental contributors to behaviour which is currently considered and treated as psychosis in persons with dementia. We developed a questionnaire which inquires about psychosocial and environmental etiologies of psychotic symptoms in dementia, and which tries to illuminate the patient's point of view. We conducted interviews with staff members about 74 nursing homes residents. In addition to the assessment of psychotic symptoms and their causes, we collected information about the demographic, medical, functional, and cognitive status of participants. As hypothesised, the understanding of the psychosocial and environmental factors lead to the conceptualisation of non-pharmacological interventions. Since we were not able to test the interventions, further research is needed to clarify the feasibility and the efficacy of these interventions. However, anecdotal findings of the study suggest that having participated in the study enhanced the caregiver's understanding of persons with dementia, and therefore had a positive impact on their quality of life. The findings, which are currently being analysed and written up, are expected to lay the foundation for a deeper understanding of persons with dementia and of the true meaning of psychotic symptoms in this population.

Цел на проекта
Background: The current estimate for the number of people with dementia in Europe is 5 million, and for the world, 24.3 million, with an estimated 4.6 million new cases every year. 30% to 50% of the people suffering from Alzheimer's Disease have psychotic symptoms. Psychotic symptoms in dementia include delusions and hallucinations. These symptoms are seen as rooted in the biological changes that cause dementia and are usually treated with anti-psychotic medication. An alternative approach to the bio-medical approach is the social-environmental approach (Cohen-Mansfield, 2003), which analyzes psychotic symptoms in dementia relative to the interaction between the person's cognitive limitations and the physical and social environment.Despite the anecdotal support for this theory, no studies have been done which have investigated psychotic symptoms from an environmental/psychosocial perspective and the applicability of this model is still unknown.Specific Aims:- Identification of psycosocial and environmental factors that seem to trigger or contribute to the manifestations of psychotic symptoms in dementia, and estimation of their relative prevalence.- Development of a set of non-pharmacological interventions based on the factors identified in aim #1.- A pilot-test of the feasibility and the efficacy of non-pharmacological interventions for psychotic symptoms in dementia.Importance: This study will provide a greater understanding of psychotic symptoms in persons with dementia and lay the groundwork for an intervention study in which non-pharmacological interventions will be used instead of psychoactive medications.By enhancing the understanding of these symptoms and potentially decreasing the negative side-effects associated with psychotropic medication, t his study has the potential of providing information that is crucial for improving the policy of care and enhancing the quality of life of persons with dementia and their caretakers.
